Resource Description: 

The QFP advises family planning providers to consider the need for STD services for all clients, including those without an STD-related “reason for the visit.” The attached tools offer guidance for conducting a sexual health assessment to help determine a client’s need for STD services.

The Conducting a Sexual Health Assessment handout offers sample statements and tips for starting the conversation and helping the client feel comfortable, along with sample questions for the “5 Ps” of a sexual health assessment: partners, past STD history, pregnancy, practices, and protection from STDs.

The Five Ps job aid briefly highlights the key topics to cover in a sexual health assessment, in a half-sheet that can be printed and easily referenced during client visits.
